@joris.roling what USB profile or USB configuration does your teensy use?
This defines the USB descriptor and the configurations / interfaces in it.
If your software runs on T4.1 you could send me the hex and I can review it. Or a demo code like @rvh did.
When you connect T4 to your mk2 and open the USB Host screen, do you see any MIDI ports there?
